				<description><![CDATA[JEFFERSON - Jessica L. Cook, age 28, of Jefferson, died Monday, May 7, 2012, at University Hospitals of Cleveland. <br /><br /> She was born May 24, 1983, to Jennifer (nee: Caruso) and Jesse Cook Sr. and lived in Rock Creek before moving to Jefferson five years ago.<br /><br /> Jessica worked as the store manager for the Shoe Dept. in Chardon. <br /><br /> She is survived by her parents; sisters, Julia Cook of Jefferson and Jennifer (Matthew) Brining of Youngstown; brother, Jesse Cook Jr. of Ashtabula; boyfriend, David Anderson of Charleston, W.Va.; maternal grandparents, Veronica and Nick Caruso Sr. of Ashtabula; and several aunts, uncles and cousins.<br /><br />A funeral service was held Monday at the Burr Funeral Home in Chardon. Burial was at All Souls Cemetery in Chardon Township.<br /><br /> Donations are suggested to the family.<br />]]></description>

				<description><![CDATA[KINGSVILLE - James M. Marinchak, age 83, of Dibble Road, passed away Tuesday, May, 15, 2012, at Ashtabula County Nursing and Rehabilitation Center.<br /><br /> He was born Nov. 6, 1928, in Cleveland, the son of George and Helen (Fodor) Marinchak and had been an area resident most of his life. After graduation from high school, Jim enlisted in the United States Army on Nov. 14, 1948 and was honorably discharged on Sept. 29, 1951 after having achieved the rank as Corporal. During his time in service, he was stationed in Korea, while there he was wounded in action. Jim received citations for his bravery. He was awarded the Purple Heart, three Bronze Stars, the Korean Service Medal, the Presidential Unit Citation, and Distinguished Unit Citation.<br /><br /> On Nov. 30, 1968, he married the love of his life, Alice Ackley and together they celebrated forty-three years of wedded bliss prior to her death on April 20, 2012. Jim and Alice enjoyed each other's company and you would never see one without the other. They loved to go to BINGO and camping.<br /><br /> After returning home, he worked as a patrolman for the Union Dock Company for many years, Jim really enjoyed his job and was disheartened when the company closed. He then worked for Obron Corporation as a maintenance person for twelve years prior to retirement. Never one to be idle; he was always tinkering. He was affectionately known as Mr. Fix-It. Jim's cat, "Wally" was his constant companion and enjoyed sitting on his lap.<br /><br /> A wonderful husband, dad and grandpa, he will be remembered for his incredible work ethic, his love of model trains and his devotion to his wife and children. Jim fought the good fight, his race was won.<br /><br /> His survivors include his four children, James R. Marinchak, Rebecca (Robert) Headley, Charles (Kim) Marinchak and Scott Marinchak; and six grandchildren, Jeremy, Zach, Dakoda, Austin, Connor and Kamryn.<br /><br /> He was preceded in death by his siblings, Irene, Margaret, John, Helen, George, Gizella, Ernest and Joseph; and his parents.<br /><br />It was his wish that no calling hours will be observed. He and his beloved wife, Alice will be interred in Western Reserve National Cemetery, Rittman, Ohio, with Full Military Honors being provided by the Joint Veteran's Council of Medina County.<br /><br /> Contributions may be made to the family in care of ZABACK-DUCRO FUNERAL HOME, 500 W. 				<description><![CDATA[ERIE, Pa. - Greta Lynn Slaviero, age 62, died unexpectedly Tuesday, May 15, 2012, at Select Specialty Hospital.<br /><br /> The daughter of Joseph and Greta (Andring) Lorelli, she was born Nov. 27, 1949, in Trentum, Pa. On Dec. 31, 1966, she married Jack Slaviero and they had celebrated forty-five years of marriage.<br /><br /> Lynn was privileged to be a homemaker, she enjoyed being able to stay home and care for her family. A wonderful cook and baker, her home was filled with wonderful smells coming from the kitchen. There was nothing she could not bake or no new recipe she would not try. Family and friends looked forward to the many gatherings at her home, knowing that she would be gracious and encourage them to eat.<br /><br /> Lynn was a determined woman, she knew what she wanted and she would persevere until her goals were reached. She encouraged her sons to go out into the world and make a life for themselves. Yet, they always knew that a warm loving home awaited them on their return visits.<br /><br /> She is survived by her husband, Jack; her three sons, Jeff (Betsy) Slaviero of Geneva and Steve and Dominic Slaviero, both of Kingsville; five grandchildren, Amanda, Amber, Ariel, Jacob and Breanna; one great-grandchild, Kyle Jr.; her mother, Greta Lorelli of Vandergrift, Pa.; siblings, Debbie Geiger and Mike Lorelli, both of Vandergrift, Pa., Mitch (Donna) Lorelli of Saybrook Township, Connie (Noel) Himes of Phoenix, Ariz. and Kathy (Timber) Petroski of Vandergrift, Pa.<br /><br />A Mass of Christian Burial will be celebrated 12 Noon, Saturday, May 19th, at Saint Andrews Church, 3700 SR 193, Kingsville, Ohio, with Father Charles Poore officiating. 				<description><![CDATA[CONNEAUT - Dean L. Horton, age 90, passed away early Wednesday morning, May 16, 2012, surrounded by family, in the home built by his father on Liberty St. in Conneaut, Ohio, where he lived his entire life.<br /><br /> He was born April 24, 1922, in Conneaut, Ohio, the son of Fred L. Horton and Myrtle P. (Lillie) Horton and had been a Conneaut resident all of his life. A graduate of Conneaut High School, he joined the Army Air Corps in 1942, was commissioned as a Lieutenant in 1944 and served until 1946, then joining the Army Reserves. He was reactivated during the Korean Conflict and then returned to the Ohio National guard, serving as Commander of "A" and "C" Company for many years. He was in uniform in three different wars, received three Letters of Commendation from Major Generals, retiring from military service in 1982 as a Major.<br /><br /> Prior to his retirement in 1982, he worked in the Lab of General Tire and Rubber in Ashtabula.<br /><br /> A lifelong member of the Congregational Church, he loved the Lord and had an intimate relationship with his Savior Jesus Christ making his spiritual side a constant in his life. He was a member of the Longaker Sunday School Class; Church Council, serving almost every position; president of Church Men's Fellowship; chairman of the Church Sesquicentennial; and treasurer of the Full Gospel Businessmen's Association. He traveled with a Mission team which built a church for the Lajoya Indians in Central Mexico. He spent two seasons, nine weeks each, at the Lachish Archeological Dig in Israel.<br /><br /> He served the Conneaut Little Theater as secretary. He built or worked on the sets for several musicals by the MacDowell Music Club, including a revolving stage for the 18 scene changes for "My Fair Lady". This set was also used for the Miss Ohio Beauty Contest. He was treasurer of the Lions Club; committeeman of the American Legion; served several terms on City Council; was responsible for getting Conneaut's designation as a Tree City USA; chairman and treasurer of the Tree Commission; served twice on the Charter Commission; committeeman of the Historical Society, Scoutmaster and Assistant Scoutmaster of Troop 34, as well as committeeman; chairman of the Artists Guild; served on the 1948 Sesquicentennial Committee; served on the City and County Republican Committee and as a member of the Presidential Task Force; taught carving at the Great Lakes Woodcarving School for 25 years; set up carving demonstrations and displays for many "Log Cabin Days" and "Covered Bridge Days"; helped rebuild the Middle Road Covered Bridge; rebuilt Ohio's oldest Fire Truck (wagon) now in a museum in Erie, Pa.; conductor on "Lolly the Trolley" for Conneaut's 200 year centennial; and Conneaut's Citizen of the Year in 1999.<br /><br /> He is survived by his wife of 70 years, Nancy I. (Hegfield) Horton; his daughter, Lynda (David) Blood of Conneaut; a foster son, Lee (Kathy) Hughlett of Conneaut; four grandchildren, Fredrick Blood of Conneaut, Jarrod (Wendy) Blood of Conneaut, Sara (Jon) Sherer of Alliance and Shawn Hughlett of Conneaut; eight great-grandchildren, Dean, Matthew, Moria and Laura Blood of Conneaut, Ian and Mark Sherer of Alliance and Dylan and Troy Hughlett of Conneaut; as well as several nieces, nephews and more distant relatives.<br /><br /> He was preceded in death by his son, Fredrick L. Horton in 1964; his brother, Myles Horton; his sister, Amy Horton Bell; and his parents.<br /><br />Funeral Services will be held 4 p.m., Saturday, May 19th, at MARCY FUNERAL HOME AND CREMATION CENTER, 208 Liberty St., Conneaut, Ohio, with his son-in-law, the Rev. David Blood officiating. Burial will be at the convenience of the family on Monday in Glenwood Cemetery.<br /><br /> Calling Hours will be from 2 to 4 p.m., Saturday, at the Funeral Home.<br /><br /> Memorial contributions may be made to the Video Ministry of First Congregational United Church of Christ or to Hospice of the Western Reserve. She also loved flowers, cats and birds.  <br /><br /> She married Lawrence M. Johnson Sr., on April 25, 1942; he preceded her in death on July 18, 1994. Also preceding her in death are her son, Lawrence M. Johnson Jr.; four grandchildren, Brian, Michelle, Brenda and Michael Johnson; and two sisters, Lois Wright and Evelyn Koivisto.<br /><br /> She is survived by her daughter, Beverly A. 				<description><![CDATA[ASHTABULA - Ann M. Lombard, age 90, of Ashtabula, Ohio, died on Saturday afternoon, May 12, 2012, at Ashtabula County Medical Center from injuries sustained in a car accident.<br /><br /> Born Nov. 16, 1921, in Erie, Pa., she was the daughter of Leonard and Eleanor (Dombrowski) Schwer. Ann grew up in Conneaut, Ohio, attended St. Mary's School and graduated from Conneaut High School, Class of 1939. She married Anthony (Lomby) Lombard of Ashtabula, in Conneaut at St. Mary's Church in August of 1947.<br /><br /> After graduating, Ann worked at the former Goldette Clothing Factory in Conneaut. She was employed by Grace Severino, her best friend, at The Import Store on Main Avenue and also at St. John's High School cafeteria.<br /><br /> An extremely devout Catholic and a member Of Our Lady of Mount Carmel Church (Our Lady of Peace), she was involved in many of the church organizations. She was a member of the Altar and Rosary Society; taught C.C.D. for ten years; initiated and coordinated Perpetual Adoration at the Church Chapel; and was involved with the annual church feast. She also faithfully visited the sick and residents of the local nursing homes in the Ashtabula area, bringing them fresh fruit to enjoy. Ann and her husband annually made the pilgrimage to Carey, Ohio, to visit the shrine. She was a member of the Secular Franciscan Order and generously contributed to many Christian charities throughout the world.<br /><br /> Ann was also responsible in assisting financially and educating Antony Vailatt to be become a priest through a seminary in Africa. After becoming a priest, Father Vailatt made a surprise visit to Ann at the Ashtabula Country Club where she resided, to thank her for her Christian beliefs and generosity making it possible for him to become an ordained priest. Father Vailatts visit completed Ann's pilgrimage and reinforced her strong belief in helping others.<br /><br /> Survivors include her son, Michael; daughter, Lenore (Ron) Lupo; grandchildren, David (Carolyn) Enricco, Ronelle (Eric) Myers and Ronald (Christie) Lupo; great-grandchild, David Enricco; and neighbor, Donna (Palumbo) Notte, who Ann was a second mother to when her mother passed away at a young age. Ann was also anxiously awaiting the birth of her second great-grandchild which is due this week to her granddaughter Ronelle.<br /><br /> She was preceded in death by her parents; her husband, Anthony Lombard; her brother, Leonard Schwer; and her sister, Eleanor Lawrence.<br /><br />Funeral Mass will be celebrated 12 p.m., Noon, Wednesday, May 16th, in Our Lady of Peace Parish, Our Lady of Mount Carmel Church, 1200 E. 21st St., Ashtabula, with burial to follow in Saint Joseph Cemetery.<br /><br /> Visitation will be from 10 to 11:30 a.m., Wednesday, at GUERRIERO FUNERAL HOME, 1550 W. 19th St., Ashtabula.<br />]]></description>

				<description><![CDATA[PHOENIX, Ariz. — John "Jack" Willard English Jr., age 65, of Phoenix, Ariz., formerly of Rock Creek, Ohio, passed away Monday, April 9, 2012, at Phoenix Hospice of the Valley.<br /><br /> Born on Feb. 8, 1947, in Oakland, Calif., he was the son of the late John and Harriet (Oltman) English. He attended Rock Creek Elementary School and was a member of the 1965 class at Jefferson High School.<br /><br /> Jack was a licensed Real Estate Broker in Arizona and owned AZ100 Realty.<br /><br /> When he was seven, he saved his four year old sister when she fell into a pond. As a child, Jack loved astronomy and spent many hours reading about it and gazing at the planets with his telescope. He traveled across the country during the Bicentennial by covered wagon.<br /><br /> He was the person to call when anyone had questions about Arizona real estate. He was noted for being honest and fair. He spoke often about his family and friends, and growing up on their farm. His great sense of humor will be missed by his family and his many wonderful friends.<br /><br /> Survivors include his children, Lonnie (Jennifer) English and Michelle (Derek) Simmen, both of Jefferson, Ohio, and Julie (Sean) McAngus of Canton, Ohio; sister, Linda (William) Kirk of Ashtabula, Ohio; and grandchildren, Marshall, Megan and Emily English, Hayden, Kennedy and Jaxyn Simmen and Taylor McAngus.<br /><br /> He was preceded in death by his parents.<br /><br />Services are pending.<br />]]></description>

"Dick" Prosser, age 75, a resident of Geneva, Ohio, passed away Sunday, May 13, 2012, at his home.<br /><br /> Born in Conneaut, Ohio, on Jan. 9, 1937, he was the son of the late Francis and Bertha (Wilson) Prosser. On August 4, 1956, in Ashtabula, Ohio, he married Joyce L. McGranahan.<br /><br /> Dick was the owner of an auto sales business in Geneva and previously had owned a car dealership in Jefferson, Ohio.<br /><br /> He was a member of Geneva VFW Post #6846 and FOE Aerie #2243. He enjoyed going to all kinds of auctions including cattle, horses and cars, showing horses, gambling and was involved in stock car racing.<br /><br /> Survivors include his wife, Joyce of Geneva; son, Richard (Regina) Prosser Jr. of Geneva; daughter, Sarah (Russ) Frain of Austinburg; six grandchildren, Matt, Garrett, Becky, Russell Jr., Erica and Sarah; four great-grandchildren, Darrian, Alexander, Aaron and Amelia; sisters, Lila Muto of Painesville, Joyce Lee of Saybrook, Connie Jenkins of Geneva and Judy (Harrison) Moss of Geneva-on-the-Lake; and a brother, James Wilson of Florida.<br /><br /> He was preceded in death by a son, David Prosser; his parents; sisters, Phyllis VanTassel, Betty Curtis and Mildred Baumgardner; and a brother, Larry Wilson.<br /><br />No services are scheduled at this time.<br /><br />]]></description>

				<description><![CDATA[SAYBROOK TOWNSHIP - Francis A. Brewer Sr., age 79, of North Bend Road, Saybrook Township, Ohio, died Sunday, May 13, 2012, at his home.<br /><br /> Born August 4, 1932, in Geneva, Ohio, he was the son of Francis "Tom" and Pauline Brewer. A Veteran of the United State Army, he then was transferred to the United States Air Force when it was formed.<br /><br /> Francis was a member of the International Union of Bricklayers and Allied Crafts workers.<br /><br /> He was the owner of the first Hydro-Glide Harley-Davidson motorcycle in Ashtabula County. He built and flew different planes when he lived on Myers Road.<br /><br /> He was a member of the United States Parachute Association and made over 100 jumps'. He enjoyed sailing and raising farm animals.<br /><br /> Surviving him are his wife, Jeanette (Camp) Brewer, whom he married Nov. 8, 1958; two sons, Andrew (Patricia M. Lewis) Brewer of Rock Creek and Francis A. She married George Hubbard on May 29, 1948.<br /><br /> Eleanor had worked for many years as a teller, then assistant manager of Peoples Savings and Loan Bank in Ashtabula.<br /><br /> Eleanor was a member of Bethany Lutheran Church; past treasurer of the Harbor Coalition; past treasurer of the Bethany Lutheran Sisters; Harbor High Alumni Association; Finnish American Heritage Association; sang in the Messiah Chorus; Ashtabula Women's Club; and past "Grey Lady" at Ashtabula County Medical Center. Truran Jr., age 83, of Meadville, died Thursday, May 10, 2012, at home.<br /><br /> He was born Jan. 20, 1929, in Meadville, the son of Richard C. Truran Sr. and Julia Coon Truran. He was a graduate of Meadville High School and a Veteran of the U.S. Army from 1951-1953. Richard married Helen Gammell on Sept. 5, 1951; she preceded him in death on July 13, 2010.<br /><br /> He was the owner of Printcraft, Inc., in Conneaut, Ohio and a member of First Presbyterian Church.<br /><br /> Richard was a member of Elk's Lodge #219, P.E.R., P.D.D.G.E.R. and VFW #2006. He enjoyed golfing and spending time with his grandchildren and great-grandchildren.<br /><br /> Survivors include two daughters, Linda Miller of Meadville and Cynthia Thompson and her husband Eric of Conneaut; two sons, Richard C. Truran III and his wife Mary of Meaville and Thomas W. Truran and his wife Diane of Girard; son-in-law, Joseph Simonette of Meadville; 12 grandchildren, Michael Simonette, Matthew Simonette, Hilary Kahl, Jonathan Truran, Brian Truran, Christina Shamp, Jacqueline Beers, Lisa Miller, Jesse Thompson, Bryan Morris, Danielle Morris and Jordan Morris; nine great-grandchildren, Brandon and Allison Simonette, Nick, Alex and Ashley Simonette, Kaizen Morris, Parker Dean, Gavin Kahl and Ryder Beers; and three sisters, Charlotte Kasbee of Cambridge Springs, Louise Mushrush of Meadville and Marjorie Filegar and her husband John of Meadville.<br /><br /> He was preceded in death by his parents; wife, Helen; daughter, Marilyn Simonette; and a daughter-in-law, Sheri Truran.<br /><br />Funeral Service will be 11 a.m., Monday, May 14th, at HATHEWAY-TEDESCO FUNERAL HOME AND CREMATORY, 614 Baldwin St., Meadville, Pa. 				<description><![CDATA[SPRING, Texas — Carol Acierno Lackson was born May 2, 1939 in Ashtabula, Ohio and passed away May 5, 2012 in Spring, Texas after a battle with colon cancer.<br /><br /> Preceding her in death were her mother Ellen Acierno and father Edward Acierno.<br /><br /> She is survived by her husband Carl of Spring, Texas; daughter Dr. Susan Hutchins, son-in-law Dennis Hutchins, granddaughter Katherine Hutchins of The Woodlands, Texas; sister Roberta Knoepp and her husband David of Westlake, Ohio; sister Mary Ellen DeMarco and her husband James of Naples, Florida; sister-in-law Norma Dragon and her husband Andrew of Ashtabula, Ohio; and numerous cousins, nieces, and nephews living throughout the United States. <br /><br /> Carol graduated salutatorian from Ashtabula Harbor High School in 1957 where she was Head Majorette for two years. She received her B.A in Sociology, graduating with honors, from Baldwin-Wallace University in Berea, Ohio in 1961 which she attended on a full scholarship from General Motors.<br /><br /> She was a social worker in Cleveland, Ohio and Chicago, Illinois and in later years was a real estate broker in Houston. Although she moved from Ohio many years ago, she remained a loyal fan of the Ohio State Buckeyes. Always an avid golfer, she enjoyed membership at Medinah Country Club in Medinah, Illinois; Champions Golf Club in Houston, Texas and Lake Windcrest Golf Club in Magnolia, Texas.<br /><br /> Carol and Carl would have celebrated their 50th anniversary this June. She was a loving wife, mother, and grandmother. Dubach, age 93, of South Ridge Road West, Saybrook Township, died Thursday, May 10, 2012, at his home.<br /><br /> He was born May 20, 1918, in Ashtabula, the son of Frederick and Martha (Shipbaugh) Dubach and had been a lifetime area resident. He was a 1936 graduate of Ashtabula High School.<br /><br /> Ralph had been employed with the True Temper Corporation for 10 years and retired as an Agent for Nationwide Insurance.<br /><br /> He married Dorothy Root on June 27, 1940 and was baptized as one of Jehovah's Witnesses on Oct. 19, 1957. 				<description><![CDATA[PLYMOUTH TOWNSHIP - June Marjorie Hecker, age 86, died Monday, May 7, 2012, at her home.<br /><br /> The daughter of Thomas and Anna Walsh, she was born on June 24, 1925, in Harrisburg, Pa., and had moved from Mineral Ridge, Ohio, to Ashtabula twelve years ago.<br /><br /> On Feb. 15, 1944, she married her husband, Clyde Carl Hecker, and together they celebrated sixty-two years of marriage prior to his death on March 9, 2006.<br /><br /> June had a wonderful work ethic; she enjoyed working. She was employed as a bookkeeper for Higley Florist in Girard for more than forty years. She had a passion for her job and for those she worked with. June maintained her many friendships even after her retirement.<br /><br /> Never one to be idle or bored, she had many hobbies and interests. While married to Clyde, they traveled to many interesting places, June's favorite was French River, Canada. While in Canada, she liked to go fishing and catch Walleye. She also enjoyed telling stories of her great catches.<br /><br /> June was an avid gardener and her family and friends benefited from her "green thumb." An extraordinary seamstress, baker and cook; everyone enjoyed gathering at her home during the holidays to share food and good times. June shared her time with her two dogs "Toby and Molly" and many times she could be found reading and sharing the same chair with her pets.<br /><br /> A woman of great determination, June suffered a stroke several years ago and through hard work and many struggles, she was able to regain full use of her abilities. She will be remembered for her courage in the face of adversity, her wisdom and gentleness, her warm smile and easy sense of humor, and her unconditional love for friends and family. June loved her children and grandchildren with her whole being and gave so much of her-self to them expecting nothing in return. She will be remembered by those she loved and those that loved her.<br /><br /> She is survived by her children, Phyllis Coburn-Paris (Robert) of Plymouth Township and Dennis (Cathy) Hecker of Columbus; grandchildren, Sean Hecker and Stacy (Brandon) Miller; great-granddaughter, Kylee Marie Miller; and many loving nieces and nephews.<br /><br /> She was preceded in death by her great-granddaughter, Allison Miller on Nov. 16, 2009; her husband, Clyde; brothers, Lester, Gerald and Pete Walsh; her sister, Dorothy Eppley; and her parents.<br /><br />Funeral Services will be held at 12 p.m., Noon, on Thursday, May 10th, at the DUCRO FUNERAL HOME, 4524 Elm Ave., Ashtabula, Ohio. 				<description><![CDATA[JEFFERSON — Richard Thomas "Dick" Watson, age 80, died Monday, May 7, 2012, at his residence with his loving wife and his son at his side.<br /><br /> Dick was born Oct. 1, 1931, to Walter L. and Myrtle B. (Garwood) Watson, in Conneautville, Pa. and had moved to the Ashtabula area in 1942 as a child with his family. Dick joined the United States Army in 1949 and was honorably discharged in 1950. On July 25, 1953, he married the love of his of life, Ruthella Paine, and together they celebrated fifty-eight years of marriage.<br /><br /> Dick was a hard worker with a wonderful work ethic that he learned from his parents. He worked thirty-one years at Ashtabula Bow Socket; while he was there, he made many friends with whom he still kept in touch. Dick and Ruthella owned the Four Corners Bar in Austinburg for five and one half years. During their time as owners, they met many wonderful people. It was those people from in and around Austinburg that made it a wonderful place to own and a privilege to have served them during that time.<br /><br /> A man of many interests, Dick was fond of country and western music, not the new age country music, but true country, with Waylon, Merle, Johnny and Patsy who could sing of the times he remembered as a child. Along with western music, he enjoyed reading western novels written by Louis L'Amore and Zane Grey. Ruthella and Dick traveled throughout the United States; their most enjoyable memories are those of going out west to Yellowstone Park and Mt. Rushmore.<br /><br /> Dick was a wonderful husband, dad and uncle; he was loved by his nieces and nephews, each wanting to spend a weekend with him and Ruthella. He will be remembered for his ability to give much to others and ask for little in return, his courage in the face of illness and his undying love for his wife, Ruthella and his son, Jimmy.<br /><br /> He is survived by his wife, Ruthella; son, James "Jimmy" of Jefferson; brother, Doug and his wife, Patti Watson of Atlanta, Ga.; sister, Margaret Hopkins of North Ridgeville; and many loving nieces and nephews.<br /><br /> Dick was preceded in death by his brother, Duane Watson; and his parents.<br /><br />It was Dick's request that no services or calling hours be observed.<br /><br /> If desired, contributions in his name may be made to Hospice of the Western Reserve, 1166 Lake Ave., Ashtabula, Ohio 44004.
